Is there (or has anyone setup a custom monitor) to monitor the number of active Remote Desktop Sessions on a Terminal/RDS session host server?

You could use the Performance Monitor and watch the following performance counter:

TerminalServicesActive Sessions

Microsoft has a document about using the User Input Delay per Session counter to monitor how performant the user RDP sessions are at:
